Staples Account Action Request Form

  2. This form is applicable to:

    Miami County Employees whose duties include ordering supplies for their assigned department may submit this form to access the county's Staples account. Only active Miami County employees will be given access at the department head's discretion. 

  3. New User Account Information

    All new accounts will receive confirmation e-mails from Staples Business Advantage when they have been successfully set up. The new users will have to activate their account from the e-mail and set up their new passwords. 

    If users forget their password, they must use the forgot my password link on the website. 

    If a user should terminate their employment, the department must contact the Administration department to have this account deactivated immediately.

  4. The email address must be county-issued. Personal email addresses will not be accepted.

  8. Specify all authorized budget authorities this user should have access to when ordering office/custodial supplies from this account.
